Working collaboratively with the patients' primary care physician (PCP), CeutiCare optimizes the chronically ill patients' medications on an ongoing basis. We equip clinical pharmacists with our proprietary software to evaluate patients at the point-of-care. How It Works
Pharmacists located at the physician’s office evaluate chronically ill patients. The pharmacist monitors the patients on an ongoing basis and, during scheduled office visits, evaluates the patient and discusses medication needs with the physician. We educate the patient, encourage lifestyle changes, and recommend medication adjustments to achieve better outcomes.
Key to our service is our proprietary CeutiCare ICGTM system. CeutiCare ICG comprises our disease-state algorithms which guide the clinician toward optimal patient specific medications.
