CeutiCare puts pharmacists and physicians together to better help patients
CeutiCare has a central office in the greater Toledo area of northwest Ohio. The central office provides technical and logistical support to all CeutiCare pharmacists. However, each pharmacist works in a physician’s primary care office to interact with patients and physicians, and to provide collaborative medication therapy management (CMTM).
CeutiCare will work with pharmacists to try and place them at geographical sites of their choosing. There may be limitations as to where a pharmacist can be placed depending upon which sites are currently available, current site staffing, etc. At this time CeutiCare sites are operating in central Ohio and northwest Ohio, but CeutiCare is working on broadening its outreach throughout all of Ohio and beyond.
CeutiCare pharmacists, since they work within outpatient primary care settings, do not have to work holidays, and typically do not have to work weekends. Since CeutiCare fits well within the Medical Home Model, and since some advocates of the Medical Home believe clinic hours should be extended to include Saturdays, there is a possibility that in the future some Saturday hours may be necessary. CeutiCare has also developed an on-call system in which each CeutiCare pharmacist must participate in an effort to provide 24/7 availability of pharmacists to both physicians and patients. While some pharmacists may be asked to assist with drug sample management in select offices, pharmacists will NOT have any dispensing responsibilities.
In addition to central office support, pharmacists will receive training and certification pertaining to our clinical model and also our proprietary software. The software has two basic components; an electronic medical record (EMR) and a suite of decision-support algorithms for drug selection customized to each patient’s individual needs and characteristics. CeutiCare software is used only by our pharmacists within each practice site.
